Automations: Effortless Form Delivery

Automations allows you to automatically send necessary forms or packets to your patients as soon as they have appointment scheduled.

To start using this feature, make sure your Zentake account is Integrated with your Elation Health. If you need assistance, please contact our support team.


How it works

Set Up Your Automation:


  1. Navigate to Automations: In your Settings, go to the 'Automations' tab.

  1. Create Automation: Click on 'Add Automation' and Name your Automation.

  1. Select Form: Choose the form or packet that needs to be sent automatically to patients when an appointment is scheduled.

  1. Select Your Trigger: There are 2 triggers to chose from.
    • Scheduled Appointment (Used for intake forms or assessments for before an appointment)
    • Check Out (Used for after a patient has finished their appointment. Great for post appointment surveys or assessments.)


  1. Choose Appointment Type(s): Pick which appointment types are going to trigger this automation. (you can select one or multiple)

  1. Save and Activate: Save your automation and activate it.

Automatic Form Delivery! Once an appointment is scheduled in Elation, Zentake will automatically send the selected forms to the patient, ensuring they have everything they need before their visit.
