The Zentake Scoring Assessment feature is a dynamic tool within our form builder that lets you create custom equations to calculate scores in real time. Using this feature, you can assign different values or formulas to questions so that when respondents complete your form, their answers are automatically tallied to produce an assessment score. This functionality is especially useful for:

Medical assessments: Quickly calculate severity scores or risk levels based on symptom inputs.
Financial forms: Total costs, fees, or insurance premium calculations.
Custom evaluations: Any situation where an equation (or series of equations) needs to be applied to form data.

Setting Up a Scoring Assessment

1. Create a New Form or Edit an Existing One

Access the Form Builder: Log into your Zentake account and navigate to the form builder.

2. Add Questions That Feed into the Score

Input Fields: Number, Choice, Matrices and other Calculation forms can be added to your equations.
Assign Values: Ensure that each Choice and Matrix has value added that you want to include in your final score.

3. Insert a Calculation (Scoring Assessment) Field

Create the Calculation Field: In the advanced question types, choose “Calculation.”
Design Your Equation: This is where you create the equation. You can incorporate numbers, mathematical operators (such as +, −, ×, ÷), and reference other fields by their question names.

For example: if you have questions for the nine items in the PHQ-9 form, you could create an equation like:

q1 + q2 + q3 + q4 + q5 + q6 + q7 + q8 + q9

This formula sums the values assigned to each of the nine questions to compute the total score.

Customization: You can write any equation you need. Whether it’s as simple as summing values or using more complex mathematical functions, the scoring assessment field is completely flexible.
