Setting Up the Provider Field (Elation Integration)


To ensure the correct provider is assigned and a patient chart can be created in Elation, you can configure the provider in one of two ways.


Option 1: Let the Patient Choose the Provider

You can allow patients to select the provider they are seeing directly on the form.


Step 1: Add a Provider Question

Create a question on your form such as:

“Who is the patient here to see?”

Use a Dropdown or Single Select field and list your providers.

Step 2: Get the Provider ID from Elation Settings

To correctly link each provider, you’ll need their Elation Provider ID.

In Zentake, go to:

Settings → Integrations → Elation Settings

Locate the provider and copy their Elation Provider ID.

This step is required—each provider must use the ID listed in Elation Settings.


Step 3: Add the Provider IDs to the Integration Value

Expand the choices

Paste the corresponding Provider ID into the Integration Value field.

When a patient selects their provider, the submitted form will automatically assign the chart to that provider in Elation.


Option 2: Set a Default Provider

You can configure Zentake to automatically assign all new charts to a default provider.

Navigate to:

Settings → Integrations → Elation Settings

Step 2: Enable Automatic Chart Creation

Turn on Automatic Chart Creation.

Step 3: Select the Default Provider

Choose the provider that should be assigned to each chart that is created.

Once enabled:

  • All charts created from submitted forms will be assigned to the default provider.
  • If a chart already exists in Elation, the assigned provider will not be changed.

Disable Automatic Chart Creation

To prevent charts from being automatically created when a form is submitted, simply uncheck the “Automatic Chart Creation” setting in Elation Settings.
