Auto Creating a Chart in Elation - Setting up the provider

To ensure patient charts are created successfully in Elation and assigned to the correct provider, configure the provider using one of the following methods.

Note: Automatic Chart Creation must be enabled for either of the provider assignment methods below to work.

Option 1: Let Patients Select Their Provider

This option allows patients to choose their provider when completing the form.

Step 1: Add a Provider Question

Add a Dropdown or Single Select question to your form, such as:

Add each of your providers as a choice.

Step 2: Copy the Provider IDs

In Zentake, navigate to:

Settings → Integrations → Elation Settings

Locate each provider and copy their Elation Provider ID.

Note: Each provider must use the Provider ID shown in Elation Settings.

Step 3: Map Each Choice

Expand each provider choice in your form and paste the corresponding Elation Provider ID into the Integration Value field.

When a patient selects their provider, Zentake will automatically assign the new Elation chart to that provider.

Option 2: Assign a Default Provider

If all new patient charts should be assigned to the same provider, you can configure a default provider.

Step 1: Open Elation Settings

Navigate to:

Settings → Integrations → Elation Settings

Step 2: Select the Default Provider

Choose the provider who should be assigned to all newly created patient charts.

Once enabled:

All charts created from submitted forms will be assigned to the default provider.
If a chart already exists in Elation, the assigned provider will not be changed.

Disable Automatic Chart Creation

To stop Zentake from creating new patient charts when forms are submitted, turn off Automatic Chart Creation in Settings → Integrations → Elation Settings.
