Elation: sync allergies

Overview

This article explains how to correctly configure Zentake so allergy data collected in forms is properly sent into Elation’s Clinical Profile → Allergies section.

There are two ways to collect and send allergy information:

1. Using a Table Question — recommended when collecting only Allergy Name and Reaction.
2. Using a Repeating Block — recommended when you also want to collect Severity and Date of Onset.

Correct setup ensures allergies are stored in a structured format and appear correctly in the patient chart.

Option 1: Using a Table Question

Step 1: Create a Table Question

In your Zentake form:

1. Add a Table Question
2. Create the following two columns:

Step 2: Map Columns to Elation

Open the integration mapping settings and configure each column individually:

• Endpoint Category → History (clinical profile)
• Endpoint → Allergy Name & Reaction
• Leave Label and Date Completed Blank

Option 2: Using a Repeating Block

Use this option if you want to collect Severity and Date of Onset in addition to Allergy Name and Reaction.

Step 1: Create a Repeating Block

In your Zentake form:

1. Add a Repeating Block.
2. Add the following four questions inside the repeating block:

• Allergy Name
• Reaction
• Severity (use single choice)
• Date of Onset (use date)

The repeating block allows the user to enter multiple allergies, with all four pieces of information collected for each allergy.

Step 2: Map Questions to Elation

Open the integration mapping settings and configure each question individually:

• Endpoint Category → History (clinical profile)
• Endpoint → Allergy Name
• Endpoint → Reaction
• Endpoint → Severity
• Endpoint → Date Started

Leave Label and Date Completed Blank.

Important

When using the Repeating Block option, do not use a Table Question for the allergy information. The four questions should be contained within the same repeating block so that each allergy and its associated information are kept together.
