Designing Your Forms

Zentake’s Design settings let you customize the appearance of your forms and documents to match your practice or organization’s branding.

Update Form Design Settings

Follow these steps to style your Zentake forms:

1. Navigate to Forms.
2. Click on a form you would like to customize.
3. Select Design.
4. Update the available design settings to match your branding and preferences.
5. Save your changes.

Logo & Header

The Logo & Header section controls how your organization’s branding appears at the top of your forms.

Choose a Layout

Choose from four header layouts to control how your logo and practice information are positioned.

The available layouts include:

• Logo only — Displays your logo without practice information.
• Centered — Centers your logo and practice information.
• Logo left — Places your logo on the left with practice information on the right.
• Logo above — Places your logo above the practice information.

Select the layout that best fits the look you want for your forms.

Your Logo

Your organization’s logo is automatically pulled from your Account settings.

If you update your organization logo, the logo used in your form design will update as well.

Use the refresh option to update the logo shown in the form designer.

Logo Size

Use the Logo Size slider to adjust how large your logo appears in the form header.

The size is displayed in pixels as you adjust the slider.

Practice Information

Enter the information you want displayed alongside your logo.

You can include information such as:

• Practice name
• Address
• Phone number
• Website
• Other contact information

Use line breaks to control how the information is displayed.

Show on Downloaded PDFs

Turn this setting on to include your logo and header information when forms are downloaded as PDFs.

Turn it off if you only want the branding displayed on the online form.

Colors

The Colors section lets you customize the colors used throughout your forms.
