How to use the Handout Question


What Are Handouts?

Handouts are documents you provide to patients that include important information, instructions, or educational content.


The Handouts field allows you to attach these documents directly to your forms so patients can easily access them while completing their intake.


This helps ensure patients receive and review important information without needing separate emails, printouts, or follow-ups.


Why Use Handouts?

  • Share instructions before or after a visit

  • Deliver consent details or preparation guidelines

How It Works

  1. While creating your form, go to Advanced Fields
  2. Add a Handouts field

  3. Upload your document under PDF File

  4. Choose whether the handout must be:
    • Viewed
    • Downloaded
  5. Save your form

What Patients See

When patients reach the handout section in the form, they’ll see the document clearly presented within their workflow.

Patient Experience:

  • The handout appears as part of the form—no separate login or email needed
  • Patients can open and view the document instantly
  • They have the option to download it to their device
  • If required, they must view or download the file before continuing

This ensures patients don’t skip over important information and helps confirm they’ve had access to it before submitting their forms.

How it works


  1. As you are creating your forms. Under "Advanced Fields" Add a Handout question to your form.

  1. Upload your hand out where it says "PDF File"

  1. Choose if you want the handout to be required to be Viewed and/or Downloaded by your patient.
  2. Save your form
