Automatically Creating Patient Charts in Elation Health

Zentake can automatically create a patient chart in Elation Health when a patient completes their first form. This helps eliminate manual chart creation and ensures new patients are automatically added to Elation as soon as their intake is submitted.

Requirements

For Elation to automatically create a patient chart after a form is completed, the following information is required:

Patient's First Name
Patient's Last Name
Patient's Date of Birth
Patient's Sex at Birth
Assigned Provider

In addition, you must:

Enable Automatic Chart Creation in your Elation integration settings.
Assign a default provider in the integration settings if the provider will not be selected through the form.

Key Requirements

If these fields are mapped correctly in your form and the patient does not already have a chart in Elation, Zentake will automatically create a new patient chart when the form is completed.

The first four pieces of information are straightforward and typically captured in most intake forms. Be sure that each field is mapped to its corresponding Elation integration field.

The Assigned Provider requirement can be met in one of two ways:

A default provider is configured in the Elation integration settings and assigned automatically during chart creation.

If neither option is configured, Zentake will not be able to create the patient chart in Elation.
