Zentake Can automatically Create a Chart in Elation When a Form is Completed

For Elation to automatically create a patient chart after a form is completed, the following five pieces of information are required:

1. Patient's First Name
2. Patient's Last Name
3. Patient's Date of Birth
4. Patient's Sex at Birth
5. Assigned Provider

Key Requirements

• If these fields are mapped correctly to integrate with your form and the patient does not already have a chart in Elation, a chart will be created automatically.
• The first four pieces of information are straightforward and typically captured in most intake forms. (Make sure the integrations on these are set)
