How to Use the Measures Feature in Zentake

The Measures feature in Zentake is a powerful tool designed to help you track assessment scores over time, offering a clear picture of trends and patterns in patient progress. This guide will walk you through how to utilize the feature to its fullest potential.

What Is the Measures Feature?

The Measures feature allows you to monitor and analyze scores from assessments over time. This is especially useful for tracking patient progress in various areas, whether it’s mental health, physical therapy, or other healthcare specialties.

Getting Started

1. Add Values and create Calculations on your Form

Before you can use the Measures feature, your form must have calculations set up so scores can be tracked.

Follow the steps in our Calculations Help Article to create the necessary calculations in your form.

  1. Convert Forms to Assessments

    • To begin tracking scores, you must first lock the form for measures tracking. This will lock the form and scoring, ensuring it cannot be edited after locking to maintain the integrity and validity of the scores over time.
    • To do this, go to the form builder for the form you want to lock. Then navigate to Settings, where you will see a "Lock Form for Measures Tracking" toggle. Switch it on and confirm that you want to lock the form.
    • Once locked, each submitted form’s score will be tracked automatically.

Key Features and Benefits

1. Track Any Assessment or Score

  • Zentake allows you to track scores from any assessment and any specific item or section within an assessment.
  • Over time, these scores will be visualized in a graph, making trends easy to spot.

2. Add Scores Manually

  • If an assessment score wasn’t automatically logged, you can add it manually to keep the tracking consistent.

3. Set Threshold Levels

  • Depending on the assessment, you can define threshold levels to help interpret scores at a glance. For instance:
    • 0-4 | No or minimal depression
    • 5-9 | Mild depression
    • 10-14 | Moderate depression
    • 15-19 | Moderately severe depression
    • 20-27 | Severe depression

4. View Charts or Tables

  • You can toggle between a chart view, which displays a graphical representation of scores over time, and a table view, which provides a detailed list of scores.
  • The table view also includes a note section where you can document additional observations or insights.

5. Add Interpretation Instructions

  • At the bottom of the Measures tab, you’ll find an area to include instructions for scoring and interpreting assessments. This ensures that everyone using the system understands how to evaluate the data correctly.
