My forms are not integrating into Elation... What to do?
If a submitted form is not appearing in Elation, don’t panic—this is usually caused by a small mismatch or configuration issue. Use the steps below to quickly diagnose and resolve the problem.
Most Elation integration issues come down to patient matching or form setup. Follow the steps below in order.
1. Confirm the Integration Actually Failed
Before troubleshooting, confirm the integration did not succeed.
How to check:
- Open the patient record in Zentake
- Go to the patient’s Timeline
- Find the Integration event
You will see one of two statuses:
- Success – The form was sent to Elation
- Failed – The integration did not complete
If it shows Success, the form was sent and no action is needed in Zentake.
If it shows Failed, continue below.

2. Check for Duplicate Patients in Zentake
Duplicate patient records are the most common cause of failures.
What to do:
- Go to Clients
- Search by the patient’s date of birth
- Look for multiple records
- Merge duplicates into the record connected to Elation
After merging, retry the integration.

Elation requires an exact match.
Common problems:
- Typos or nicknames
- Additional characters (e.g. middle initials)
6. Retry the Integration
Once the issue is fixed:
- Open the patient record
- Locate the failed submission
- Retry the integration
Most integrations succeed immediately after correcting the issue.
