How to Use the Form Builder

Zentake offers an easy-to-use, drag-and-drop form builder that allows you to create an unlimited number of full customized forms. 


Zentake has tons of different question/field types depend on the type of form you are building. If you want to learn more about the different question types you can go here.


When building a form, you will be presented with a canvas that allows you to add as many questions and sections to your form as you like. As you add questions and sections to your form, more placeholders (the box with +) will appear.

Click the  Create Form button on the "Forms" page.

We have a template library you can access by clicking the dropdown arrow.


Name the Form

Add a Section Title (optional)

Click anywhere there is a plus box to add a question.

On the left side choose which to type of question to want to add to your form

Question Settings - Each question has its own question settings. This will allow you to: make it required, add help text, add choices, set it to "office use", add conditional logic etc.


Resize questions so you can make room to add multiple questions to a row.


Click New Section to add a new section to your form.


Save your changes by clicking the  Save button at the top of the page and then Preview your form to see how it will look to your patients.


Once you save your form, it will display on the "Forms" page with the rest of your forms.


Keep in mind: If you are creating a consent form you will use the "Paragraph" question type. It is used for read only text.
